I wondered if you have to change to names in autobiographies?

I need to know if i have to change the names of people if i write about my life and childhood experiences? I don't want to hurt anyone i am writing about, do i need to change the names or get permission to include them in my writings? it started of as therapy but is turning into a book so i need to know legally where i stand on this.
